ACS Subsidized Child Care Required Documents

In Order to apply for ACS subsidized child care, the following documents are required:

From all your children in your household:

    - Birth Certificates (If the child's birth certificate is not    from U.S. the Birth Certificate must be translated by      a Notary Public)
    - Social Security Cards
    - Proof of U.S. Citizenship or permanent Residence

From the parent or legal Guardian:

    - Identification (State ID, Driver License, Passport,           Food Stamps Card w/picture)
    - Proof of Address (Utility Bill, Lease, NY State ID, NYC     ID, Cell Phone Bill)
    - Social Security Card (If available)
    - Proof of Income:
    . If looking for work, Parent must complete the        form ECE-026 Work Search Activity.
    . If receives weekly paystubs the last 12                consecutive paystubs
    . If receives bi-weekly paystubs the last 6             consecutive paystubs
    . If receives cash a completed 1069 Employment      Verification Form
    . If is currently attending school or vocational        training, a completed 1082 verification Form

Notes: 
- Self-employed parents must submit their current 1040 and appropriate schedules (ex. Schedule C, SE, K-1, etc.) along with a self-employed attestation form.
- Parents must be earning minimum wage of $15.00 per hour or more, also working or attending school for a minimum of 20 hours per week.

- If is a two-parent household, the documents are required from both parents.
